Job Description

This is a teleworking position, with the expectation that you will be visiting clients in the community setting in the Howard County + Baltimore region.

Who We Are

The Coordinating Center has 40 years of experience supporting children, youth, adults and seniors with complex medical needs and disabilities statewide. Deeply committed to a person-centered approach and philosophy, The Coordinating Center helps individuals of all ages and abilities achieve optimal quality health, affordable healthcare and meaningful community life.

Over the past four decades, we have become an industry expert in the delivery of person-centered, community-based care coordination services and population health. With a rich history of innovation, The Center has translated national movements into ground-level, community-based programs including Medicaid rebalancing initiatives, aging in place, transitions of care, population health, and housing first. We are advocates for policy and system changes that positively affect the lives of people with the most complex needs.

Who You Are

We look for people who have a passion for helping others in your community, and who align with our mission, vision, and values as a nonprofit organization. If you have experience in case management or any human services experience, this might be the organization for you! We look for collaborative and creative minds, self-managing organizational skills, and the desire to challenge and grow professionally. A bachelor’s degree required, along with at least one year of experience working with people with autism spectrum disorder.

What You Will Do

Our Service Coordinators play a significant role in the lives of individuals with autism spectrum disorder. The Service Coordinator supports the mission of The Coordinating Center by providing person-and family-centered coordination services that lead to increased independence and inclusion in the community. Service Coordinators are responsible for helping parents/caregivers with the Autism Waiver (AW) application process, completing contract requirements, documenting their activity, and assisting with transitioning out of the AW when appropriate. The Autism Waiver team is collaborative and uses the combined expertise of all team members to provide quality case management to all participants and their families.

As a Service Coordinator you will:

  • Secure information, such as behavioral, educational, psychological, and social factors, which contribute to a participant's situation and evaluate these to assist in the coordination of services and resources.
  • Complete monthly contacts with the participant’s family reviewing service utilization and requesting feedback about quality of services provided to their child/young adult.
  • Conduct quarterly in-person visits with participants in the community setting including schools, after-school programs, and the home.
  • Facilitate annual home visits with the participant and family members reassessing program eligibility and creating a plan of care.
  • Maintain contact with community providers to assist with the monitoring and coordination of Autism Waiver services.
  • Coordinate with the Local School System (LSS), the Maryland State Department of Education (MSDE), and the Maryland Department of Health (MDH).
  • Provide families with community resources information as needed.
  • Complete Autism Waiver documentation in MSDE’s online system and other systems in a timely manner.
  • Manage multiple priorities and tasks with flexibility to enhance service coordination services.
