Job Description

Position Purpose: Perform care coordinator duties to assess, plan and coordinate all aspects of behavioral and supporting services across the continuum of care for select members to promote quality, cost effective care. Serve as lead within department.
  • Onboard and provide initial assessment to members with potential care management need.
  • Document outreach, onboarding and assessments along with other care coordination that goes along with getting a member engage into care management.
  • Provide behavioral health support to Case Managers with scheduling appointments, addressing social detriments of health, connecting to resources and additional needs, support and outreach.
  • Complete trainings, in services and other educational seminars to stay in compliance with their role within the company and keep their license up to date.
  • Participate as an active preceptor and services as team leader/mentor to team members.
  • Assist with team reporting and preparation for audits.
  • Participate in process improvement workgroups and other special projects as assigned.
Education/Experience: Bachelor’s degree in Clinical psychology, social work, behavioral health or related field or equivalent experience. 2+ years of experience with Case Management work, social work or related experience.Pay Range: $25.97 - $46.68 per hour
