Responsibilities
- Identifies and resolves barriers that hinder effective care
- Ensures client is progressing towards desired outcomes by continuously monitoring care plan
- Review and record patients medical histories
- Keep records of patients progress, or lack thereof
- Be able to relate with patients and their families
- Make necessary patient appointments
- Conduct monthly face to face visits with assigned clients
- Transportation of clients as needed
- Adhering to managers directives

Qualifications
- Reliable vehicle and appropriate insurance for home visits
- Must have a Valid driver’s license and the ability to travel within the state, as required
- Experience working in home settings with populations of diverse cultures and ethnicities preferred
- Able to communicate well verbally and in writing
- Comfortable with computers and experienced in writing detailed notes
- Strong organization skills

Work Setting: Hybrid Position (work from home days, monthly face-to-face client visits)
Location: This position requires client facing visits within an assigned region across Indiana.
