We are offering case management services through the Community First Choice (CFC) and the Developmental Disability Administration (DDA) programs, where our cases managers assist our participants with the coordination and continuum of services.

Required qualifications:

  • One year of experience in Human Services case management preferred.
  • Bachelor’s degree in Human Services.
  • Must reside in the state of Maryland.
  • Possess a valid Driver's License, and reliable transportation.
  • Must demonstrate computer knowledge and proficiency in all Microsoft Office applications including but not limited to Excel, Word, Outlook.

Case Managers Responsibilities

  • Implementing training knowledge to assess a participant's medical, physical, and environmental status, the case manager assists the participants with the coordination of services to meet their individual needs.
  • Communicate with the participant, family member(s) and provider(s) to ensure the continuum of services as indicated in the plan of care.
  • Continuously assess and monitor services to assure the best quality.
  • Ability to meet required documentation.
  • Comply with State, Federal and Contract guidelines to meet all program compliances.
