Case Manager Intern
Duration: 3 – 6 months
SYNERGY HomeCare of Warwick is seeking a compassionate intern to work closely with the administrator and nurse to gain experience leading a team of caregivers and conducting client intake. Position Overview: As a Case Manager Intern, you will play a crucial role in supporting our organization's intake process for new clients.
This internship offers hands-on experience in the field of home care and provides an opportunity to learn about client assessment, intake procedures, and administrative tasks within a dynamic and supportive environment.
This position is remote and unpaid (with a gas card incentive for travels).
Key Responsibilities:
• Assist in the intake process for new clients, including gathering initial information, conducting screenings, and documenting client demographics and history.
• Communicate with clients and their families to collect necessary information and provide guidance on the intake process.
• Collaborate with clinical staff and other team members to ensure a smooth transition for clients into the program.
• Maintain accurate and confidential client records in compliance with organizational policies and regulatory requirements.
• Assist in scheduling appointments and coordinating logistics for client assessments and evaluations.
• Participate in team meetings and training sessions to enhance knowledge and skills related to intake procedures and client support.
• Provide administrative support, including data entry, filing, and organizing intake related documents and materials.
• Contribute to ongoing process improvement initiatives to enhance the efficiency and effectiveness of the intake process.
Qualifications:
• Currently enrolled in a bachelor’s or master’s degree program in social work, healthcare administration, or a related field.
• Strong interpersonal and communication skills, with the ability to interact effectively with clients and colleagues.
• Excellent organizational skills and attention to detail, with the ability to manage multiple tasks simultaneously.
• Ability to maintain confidentiality and handle sensitive information with discretion.
• Proficiency in Microsoft Office Suite (Word, Excel, Outlook).
• Previous experience in a healthcare or social services setting is a plus but not required.
