Hiring: Case Manager, Community Programs

Location: Lehigh Valley, Pennsylvania 18102

Abraxas Lehigh Valley Community Programs is hiring a Case Manager to maintain a caseload of youth in need of additional community and in-home support involved in Children & Youth or Juvenile Probation.

This role requires daily travel throughout the Lehigh Valley in both Lehigh and Northampton Counties as the Case Manager will be responsible for arranging and conducting home visits with youth and/or their families and developing and implementing appropriate service plans. As a Case Manager, you will support clients and their families in completing court-ordered conditions such as assessments, community service, schooling, and counseling. In addition, in this role, you will develop and maintain community resource guides for clients and utilizing group curriculum to facilitate group sessions.

As a Case Manager, you will supervise client progress and prepare required reports on a daily, weekly, and monthly basis. Key areas of responsibility include working on truancy prevention, addressing parent-child conflicts, supporting at-risk youth, providing parenting guidance, and handling various case management needs. Strong organizational and communication skills are required.

Salary: $23.00 per hour

Case Manager Basic Primary Duties and Responsibilities:

  • Maintains a client caseload with appropriate service plans.
  • Enforce program/facility norms, rules, and behavioral standards.
  • Coordinate and develop appropriate service plans through the utilization of various identified resources to assist the client in making the transition from a residential placement/institution to the community.
  • Supervise client activities and prepare required reports of the behavior and progress of clients, daily, weekly, and monthly reports are required as well as month end reports, incident, and arrest reports as needed.
  • Support client and their families in completing any court ordered conditions) i.e. restitution, community service and specific counseling needs and assessment.
  • Coordinate efforts with the courts, probation department, schools, employers, job training, recovery programs, counseling, family or other social service agencies and community programs to meet the client's individualized needs.
  • Maintain regular contacts with referral agency staff assigned to each client in order to maintain communication and consistency with client and family treatment to meet contractual/licensing obligations and ensure regulatory compliance with outside referral agencies.
  • Develop and maintain resources for clients including a community-based resource listing and curriculums.
  • Maintain contact with client at institutions/placements prior to release to the community.
  • Attend and act as advocate and/or liaison to all client court hearings, school meetings and home passes.
  • Arrange and conduct home visits with the client and client's family as determined by program Tier System. Meet required guidelines for number of hours spent with each client and family.
  • Provide information, training or workshops to external agencies and funding personnel regarding the program /facility missions statement and treatment philosophy.
  • Provide transportation for clients as needed for program/facility related meetings and activities as needed.
  • Develop a weekly work schedule to best service each client. Coordinate with clinical staff regarding optimum client services. Arrange and conduct visits with the client and family at the facility, home and community-based locations before and after release.
  • Support the Abraxas philosophy and missions and promote the Seven Key Principles of care.
  • Demonstrate appropriate use of Safe Crisis Management techniques and skills.

Case Manager Hiring Requirements:

  • Bachelor's Degree in a related field and one year of experience working with youth in a community based or residential setting.
  • At least twenty-one (21) years of age.
  • Background checks (Criminal, Federal, State, and, if necessary, child abuse).
  • Pre-employment physical exam and drug screen.
  • Valid driver's license from employee's state of residence.
  • Valid registered vehicle insurance.
  • Ability to work with computers and the necessary software typically used by the department.
  • Ability to work non-traditional hours such as evenings and weekends to meet clients and family's needs.
