We are currently hiring three Consultation Services Specialists to join our team. The hours for this position are Monday-Friday 8:00am-4:30pm. This is a remote position, with the possibility of periodic days/projects needed in our Eagan office.

The Department of Human Services (DHS) is in the process of replacing Personal Care Assistance (PCA) and Consumer Support Grant (CSG) services with Community First Services and Supports (CFSS). As the newest self-directed option, CFSS is designed to offer participants more choice, control, and flexibility with the services and supports needed to help them live and work in the community. Anyone who utilizes CFSS services must go through a Consultation Services Provider who is contracted with the state of MN to provide these services. The Consultation Services Specialist is responsible for understanding and implementing all CFSS program requirements.

Responsibilities:

Understand the CFSS program and clearly communicate details of the program to participants and/or responsible parties (RP), representatives, families and lead agency personnel

· Service set-up and annual CFSS orientation, which includes:

o Educating the participant/RP on CFSS, its service options, and how to make informed decisions about how to meet the participant’s needs

o Supporting the participant/RP with writing their service delivery plan to the extent the participant desires

o Reviewing the participant’s service delivery plan

o Offering guidance on whether CFSS service delivery plans are complete and only contain covered services

o Providing ongoing support as needed

  • Ongoing consultation and skill development, which includes:
  • Responding to the participant/RP’s questions/concerns throughout the service plan year
  • Supporting the participant/RP with making changes, if needed
  • Ongoing support for the participant/RP if they use the budget model

· Quality assurance support, which includes:

o Semi-annual review for the participant if they do not have a case manager/care coordinator and their spouse or parent (if a minor) serves as their worker

o Assisting the participant/RP if they are using the budget model and not fulfilling their employer duties

o Recommending to the lead agency to remove the person from the budget model, if necessary (ie. AC for a waiver/Alternative Care, DHS if the person is not on a waiver)

  • Work together with the Consultation Services team to provide excellent participant-focused support and timely service
  • Stay current on CFSS Program changes and updates, and assist the Consultation Services Lead Employee and team with developing or carrying out new or changing policies and procedures
  • Report to Consultation Services Lead Employee when problems arise, and assist with resolutions based on an understanding of the CFSS program requirements, guidelines and regulations

· Directly report to the Alliance Consultation Services Lead Employee, may also respond to the Board of Directors as required

Qualifications:

· be at least eighteen (18) years of age

· be able to pass a MN Department of Human Services background check

· Experience: have at least two years of experience in one or more of the following fields:

§ Consumer directed community support (CDCS) support planning

§ Support broker

§ Case management or care coordination (refer to CBSM – Waiver, AC and ECS case management)

§ Consumer education provided to people using self-directed services, either under a financial management services (FMS) provider enrolled with DHS or a PCA provider agency enrolled with DHS.

· Education:* meet any ONE* of the following education requirements:

1. Be a doctor of medicine or osteopathy in the state of MN

2. Be a registered nurse in the state of MN

3. Have Bachelor’s degree or higher from an accredited college or university in one of the following fields:

§ Occupational Therapy

  • Physical Therapy
  • Psychology
  • Social Work
  • Speech-Language Pathology/Audiology
  • Professional Recreation
  • Professional Dietitian
  • Sociology
  • Counseling
  • Special Education
  • Rehabilitation Counseling
  • Other Human Services field

4. Have a minimum of one year of experience working directly with people who have an intellectual disability or other developmental disability, AND have an Associate’s Degree or higher from an accredited college or university in one of the following fields:

§ Human behavior (ie. psychology, sociology, speech communication, gerontology)

§ Human skill development (ie. education, counseling, human development)

§ Humans and their cultural behavior (ie. anthropology)

§ The human condition (ie. literature, art)

§ Any other study of services related to basic human care needs (ie. rehabilitation counseling)

5. Have additional experience that meets at least one of the following:

§ One year of full-time experience providing direct services to people with disabilities or people older than age 65

§ Experience coordinating or directing services for people with disabilities or people older than age 65, including self-directed services

§ Experience coordinating their own services

Candidates must be:

· Strongly skilled in customer service and interpersonal communication skills

· Able to work autonomously while still placing value and importance on teamwork and coordination of services and information

· Strong with decision-making, problem solving, and reasoning skills

  • Strong with writing and reading skills
  • Strong with office-based computer software proficiency, including Microsoft Office programs
