CCR has a part-time position open in the following coverage areas within Butler County: Mars, Gibsonia, and Valencia!


Early Intervention Service Coordinator Minimum Requirements

  • Bachelor’s degree with successful completion of 12 college credits in sociology, social welfare, child development, early childhood education or special education, or other related social services.
  • At least one year’s experience working with families and individuals with disabilities.
  • ACT 33/34 and FBI Clearances, valid driver’s license and proof of car insurance coverage must be provided prior to hire. First Aid, Infant CPR within 4 months of employment and annually thereafter.
  • Must maintain strict confidentiality and be able to exercise sound judgement, be well organized and have excellent time management skills. Must be competent in computer usage and able to learn agency software.

Service Coordinator Responsibilities

  • Provides explanation of Early Intervention and its services to families.
  • Coordinates and assists in scheduling assessments and meetings associated with Individual Family Service Planning (IFSP).
  • Contact 100% of referrals within 2 business days of receiving initial referral sheet.
  • Complete intake for 100% of interested families, including information, releases, consents, and permissions to be submitted to clerical staff within 2 business days for processing and forwarding to providers.
  • Ensure quality of services to the family.
  • Complete monitoring visits with each family in accordance with minimum OCDEL regulations, or more as necessary, to ensure compliance with IFSPs, family satisfaction and progress. This includes reviews and updates to the IFSP and review of Provider session notes.
  • Ensure that 100% of children have transition planning and that it is completed on time.
  • Identify and locate community resources, activities and supports for the family in accordance with their priorities. Make referrals to these services and document on the IFSP and on Service Notes.
  • Provide information to access the Family Survey to each family, annually.
  • Complete all discharge paperwork to close a case within 2 weeks of discharge or before the child’s 3rd birthday and submit it to clerical staff.
  • Attend all the training required and complete a minimum of 24 hours per fiscal year.
  • Maintain a professional appearance and presentation both in the office and the community.
  • Demonstrate teamwork both with co-workers and members of the community.
  • Demonstrate initiative and flexibility in completion of job duties.

Pay: $21.54/hr.

Schedule

  • Part-time
  • 28 hours up to 35 hours/week
  • M-F, no weekends
  • No on-call
  • Flexible work schedule

Work Location

  • Hybrid through Orientation process, remote thereafter
  • In person community visits and meetings are required
