Child and Family Connections of Central Illinois is the regional intake agency for children birth to three and their families to enter the Illinois Early Intervention System. Early Intervention (EI) credentialed Service Coordinators work with families to develop an Individualized Family Service Plan and select service providers to meet the individualized needs of their child. Child and Family Connections can also assist with Social Emotional Consultative Services and free developmental screenings through the Local Interagency Councils.

JOB DESCRIPTION:
The EI Service Coordinator is responsible for accepting referrals, managing the intake process, identifying needed EI services, and making appropriate referrals for those services. The EI Coordinator informs parents/guardians of family rights related to EI services and Early Childhood Services, and advocates for services as required. The EI Coordinator is responsible for acting as a neutral entity and must take the family choice, provider expertise, timeliness of availability, location of services, and other EI requirements into consideration when facilitating referrals for EI services. Furthermore, the EI Coordinator must enter required data into Cornerstone System, facilitate team meetings for determination of program eligibility at required intervals, issue authorizations for services, and provide appropriate referrals throughout the client’s eligible period. This position is entirely grant-funded through the Illinois Department of Human Services, renewed annually, and is subject to any related changes.
