Job Description

Bezalel Care Management is seeking an NHTD/TBI Service Coordinator to provide Service Coordination to individuals within the Traumatic Brain Injury (TBI) Waiver and Nursing Home Transition and Diversion (NHTD) Waiver programs living in New York City.

SPECIFIC DUTIES:

*Must have experience completing service plan .

Our NHTD department aims to provide seniors in need of high-hour care with the support they need to prevent nursing home placement

We are seeking Service Coordinators to help provide case management-style services to the 25 patients assigned to their caseload. When describing the role in a few simple sentences, we tend to divide it into two categories; the task based responsibilities and the retention and problem-solving based responsibilities.

Task-based responsibilities are predictable and process-driven, such as ordering medical supplies, scheduling transportation, and completing paperwork. Retention-based responsibilities involve addressing patient concerns and resolving issues as they arise, requiring effective problem-solving and de-escalation skills to maintain a stable caseload.

Homecare Service Coordinator Responsibilities and Requirements:

  • Maintaining communication with all patients on a monthly basis, or more as needed
  • Maintaining good relationships with the staffing coordination team
  • Ensure all patients have home visits completed quarterly
  • Compliance with paperwork completion in accordance with due dates and timelines.
  • Proper utilization of internal systems
  • Fulfilling patient's requests for additional resources such as supplies and transportation, amongst others.
  • Bachelor's Degree required

Homecare Service Coordinator Schedule

  • On call Monday to Friday 9am-5pm for patient assistance
  • Expected weekly hours - 25

Job Types: Full-time, Part-time

Up to Pay: $50,000.00 per year

Expected hours: 25 – 35 per week
