Job Description

We are seeking a dedicated and experienced Nurse Practitioner to join our team as a Wound Care Specialist. As a Nurse Practitioner specializing in wound care, you will be responsible for providing comprehensive wound management services to patients across various healthcare settings. You will collaborate closely with physicians, nurses, and other healthcare professionals to develop and implement effective treatment plans tailored to each patient's unique needs. This role requires a high level of clinical expertise, excellent communication skills, and a compassionate approach to patient care.

KEY RESPONSIBILITIES

  • Assess, plan, implement, and evaluate care plans for patients with acute and chronic wounds, including surgical wounds, pressure injuries, diabetic foot ulcers, and venous insufficiency ulcers.

  • Utilize evidence-based practices to direct and provide wound care treatment and to promote healing.

  • Collaborate with a multidisciplinary team, including physicians, physical therapists, and dietitians, to develop and implement comprehensive care plans.

  • Educate patients and their families on wound care management and prevention strategies.

  • Monitor all phases of wound care treatment, including debridement, cleansing, dressing, and vacuum-assisted closure devices.

  • Document patient care services by charting patient and department records.

  • Participated in research and quality improvement initiatives related to wound care.

  • Maintain up-to-date knowledge of wound care nursing practices and standards.

  • Manage patient compliance through education and communication.

  • Effectively and accurately maintain all patient records and documentation; assess and record patient’s progress.

  • Effectively communicate with patients regarding their treatment and provide education to further assist patients with their healing.

  • Ensure knowledge of proper billing codes.

  • Responsible for maintaining clinic supplies for treatments.

  • Understand and follow work rules and procedures.

  • Work with the Medical Director to ensure all medical issues are addressed.

WORKING CONDITIONS

Variable Work Environments: Our providers work in a variety of settings, including patients' homes, nursing homes, and outpatient clinics. Each environment presents its own set of conditions, from navigating small, personal spaces in homes to adapting to different clinical settings. This requires flexibility and adaptability in working under various conditions and with different levels of resources.

Travel: A significant portion of the job involves traveling from one patient location to another. This could mean navigating urban, suburban, or rural areas, often requiring the nurse to drive in various weather conditions and manage their schedule to accommodate travel time between appointments.

Autonomy and Independence: There may be times when our providers work independently, making critical decisions on the spot without immediate direct supervision. This requires a high level of clinical judgment, problem-solving skills, and confidence in one’s nursing practice and knowledge of wound care.

Exposure to Different Health and Safety Environments: Working in patients' homes and different care facilities means encountering varied hygiene and safety standards. Our providers must be prepared to work in environments that may not be as controlled as a hospital setting, requiring them to take precautions to protect themselves and their patients from potential health risks.

Flexible Scheduling: The nature of mobile wound care often requires nurses to have flexible schedules to meet the needs of their patients. This might include early mornings, late evenings, weekends, and sometimes being on call. The nurse must manage a schedule that accommodates the unpredictability of patient needs and travel times, all while providing timely and effective care.

Compensation is on a per patient visit basis
