This position is responsible for all programmatic documentation, including assessments, Individual Service/Program Plans (ISP/IPP), behavior plans, goal tracking, periodic reviews and quality assurance reviews.
Minimum of one year experience working with individuals with developmental disabilities and/or mental illness in a group home/home health setting.
Must posses a Bachelor's Degree in a human services field such as psychology, sociology, social work, etc.
Must be proficient in CHRIS, DELTA, and all other QDDP related documentation tools frequently used.
