Job Description


Responsible for monitoring vital signs remotely and calling the patient to assess need for intervention by their healthcare provider or whether further monitoring is appropriate

Building relationships with patients for engagement, support and communication

Remotely monitoring enrolled patients through daily review of electronic patient data

Providing education and other tools to support improved patient outcomes

Communication between the provider and other members of the care team, as needed

Initiates phone contact with eligible Medicare patients in attempt to enroll them in CCM/RPM programs on behalf of physician practice

Completely and accurately educates eligible patients on the benefits of RPM/CCM programs


College Degree; LPN/CNA/Medical Assistant certification optional

Previous RPM/CCM care management experience

2- 5 years of related professional or healthcare medical experience

Enjoys fast paced work environment with a positive attitude

Self-motivation and the ability to work independently and with teams

Proficient in the use of Excel

Excellent oral and written communication skills

Proficient in managing multiple tasks at the same time

Experience in using EHR systems

Obtains consent from patients electing to opt-in to the programs

Troubleshoot devices and report malfunctions

Answer routine inquiries and relay information and messages to and from appropriate health professionals

Job Type: Full-time

Pay: $20.00 - $25.00 per hour
