Job Description

Overview


Responsible for the timely coordination of the referral process including the review/response/coordination of care for patients referred to Hospice for evaluation/admission and/or transfer process of patients to the hospice care center. Supervises LPN clinical coordinator to ensure all complex critical situation or concerns are elevated appropriately.



Responsibilities
  • Receives and reviews patient referrals to hospice services, ensuring compliance with applicable coverage criteria.
  • Coordinates the evaluation/admission process.
  • Supervises the team and provides oversight of IV medication, specialized equipment, and clinical complexity.
  • Communicates pertinent patient information to the patient's physician, payor source and/or appropriate care center staff via approved processes.
  • Utilizes clinical expertise and judgment when reviewing and assessing referrals for appropriate care setting and communicates with care center, Business Development, and referral source when appropriate.
  • Communicates and follows up with Business Development regarding pending admissions as needed or directed.
  • Makes referrals to care center staff and other community resources as appropriate.
  • Obtains verbal orders on all patients being referred and documents orders on appropriate referral intake documents.
  • Coordinates with Admissions department for insurance verification and authorization.
  • Meets productivity expectations with quality documentation of complete referrals including all required follow up.
  • Keeps abreast of nursing trends and knowledge.

  • Complies with policies, procedures and regulatory mandates including but not limited to abiding to the terms of the Amedisys Compliance Program.
  • Performs other duties as assigned.


Qualifications


Required-

  • Current, unencumbered license to practice as a Registered Nurse specific to that state the employee is assigned to work by the company and a minimum of one year (1) of nursing experience.
    • Nurse Licensure Compact (NLC) documents the following: The mutual recognition model of nurse licensure allows a nurse to have one license (in his or her state of residency) and to practice in other states (both physical and electronic), subject to each state's practice law and regulation. Nurses must legally reside in a NLC state to be eligible to have a multi-state license.
  • Must be familiar with general use and functions of the computer, such as, usernames and password concepts; internet; e-mail; navigation of computer desktop, including starting programs, using files, and windows, effectively use navigation buttons and tool bars; ability to self-manage online HR services and online training programs.
  • Reliable internet service and ability to office from home with minimal disruption to the workday.

Preferred-

  • Licensure in additional states in assigned territory and not covered under NLC.
  • Hospice experience, with familiarity of LCDs.
  • Two (2+) years of administrative experience in a healthcare environment.
