Virtual RN - Remote Patient Monitoring

FT Day Shift (Hours: 8a-4:30p - Remote, or work from home with 1-2 times a month in office)

ChristianaCare Center for Virtual Health is hiring a Virtual Registered Nurse (RN) who is independent, self-directed, and passionate about patient care.

The digital RN cares for patients virtually within the scope of the nursing practice license. Leveraging a variety of technology tools and software, the digital RN is responsible for the clinical monitoring and care for the patient including, but not limited to:

  • clinical assessment
  • evaluation of clinical information that is being monitored and collected from patients
  • managing clinical protocols
  • assessing psychosocial needs
  • Recommending services for the patient to the provider (i.e home health, DME, rehab services),
  • creating collaborative care plans
  • performing case management functions and escalating clinical concerns to a provider following established protocols.

Education & Requirements

  • DE RN licensure or eligibility for RN Compact licensure.
  • BSN required.
  • Certification in Case Management through the Commission for Case Management (CCM) within 18 months of eligibility is required.
  • Two years of experience as an RN, primary care experience preferred.
  • Basic Life Support (BLS) preferred.

PHYSICAL DEMANDS:

Standing, sitting, walking, bending, reaching, utilizing a keyboard, mouse, and computer technology for long periods of time, and lifting are all activities listed on the Physical Demands checklist.
